BENEFIT OPTIONS

WHO IS ELIGIBLE

All active, regular teammates who are classified to be in a benefit eligible position and who work 20 or more hours per week are eligible for benefits. You can also enroll your eligible dependents (when dependent coverage is available) as defined below. Eligible dependents are also defined in the Summary Plan Description.

Eligible Dependents include: Legally married spouse, natural children, stepchildren, adopted children, children for whom legal guardianship has been awarded, grandchildren who reside with you and are financially dependent upon you, and children who are subject to a Qualified Medical Child Support Order to age 26. Unmarried children who are incapable of self-sustaining employment can be covered regardless of age.

MAKING CAREFUL CHOICES

You have 30 days from the date of a life event, including your initial date of hire, to make your benefit choices– Teammates failing to elect coverage within 30 days will be deemed to have voluntarily waived eligibility for the respective coverage for the entire calendar year. Please choose your benefits carefully, as the elections you make for 2026 will remain in place for the plan year (through December 31, 2026) unless a “Qualified Status Change” occurs. Such changes include: marriage, divorce, death of a spouse or dependent, birth or adoption of a child, change in employment status or health coverage or your dependent no longer qualifying as an eligible dependent.

MEDICAL COVERAGE

The two plans Ascentek offers are distinctly different from one another. You decide what best fits you and your family.

UNDERSTAND YOUR COSTS
Knowing how the costs related to your benefits work with your plan will help you avoid unexpected charges.

  • Premium: How much is deducted from your paycheck to pay for the plan.
  • Deductible: The annual amount you’re responsible to pay for care before your plan helps cover costs, not including your premium.
  • Embedded Deductible: Each family member has an individual deductible, but the family also has a maximum total deductible if multiple family members need medical care during the year. Meaning, if an individual in the family reaches their individual deductible before the family deductible is reached, the plan begins to pay for services for that member.
  • Copay: A set amount you pay each time you visit the doctor or get a prescription.
  • Coinsurance: A percent of the bill you pay. Your plan covers the rest.
  • Out-of-Pocket Maximum: The most you’ll pay for covered care each year.
NEW! All preventive cancer screenings are covered at 100% (no deductible) on Ascentech’s medical plans even when a diagnosis is made.

HEALTH SAVINGS ACCOUNT (HSA)

Health Savings Account (HSA)

If you enroll in the $3,400 High Deductible Health Plan, you are eligible to enroll in a Health Savings Account (HSA) with HealthEquity. You may contribute towards your HSA using pre-tax deferrals taken directly from your paycheck and deposited into your account. You can use this account to pay for eligible medical expenses or allow your funds to accumulate for future expenses. These funds roll over from year to year.

Logging Into Your Portal the First Time:

  1. Go to my.healthequity.com, click “Create username and password”.
  2. Enter the information requested on the “Find your account” screen.
  3. Enter the information asked for on the “Verify your identity” screen.
  4. On the “Set up your login” screen, pick a username of at least six characters with numbers and letters. Choose a password of at least eight characters with an uppercase letter, a lowercase letter, and a number.
  5. Follow password creation recommendations as listed in the log in screen.
  6. On the “Your email settings” screen, enter your email address.
  7. Click the box to agree to the terms of the web site and save the agreement.

Here’s an Example of How an HSA Works:

  • During your plan year, you put $1,000 in your HSA. This money is not taxed!
  • Throughout the year, you and your family spend $400 on medical expenses. You pay using your HSA.
  • $600 is left in your HSA at the end of the year.
  • Next year, you deposit another $1,000 into your HSA. You now have $1,600.
  • If you don’t use it all up this year, you can rest easy knowing it will be there for next year.

Rules and Restrictions:

  • HSA funds can be used for eligible medical expenses incurred by you, your spouse, or dependents.
  • Eligible expenses are those defined by the IRS and include things like copayments, coinsurance, deductibles, prescriptions, dental and vision care.
  • You cannot use HSA funds for insurance premiums, except in specific cases like COBRA or long-term care.
  • HSA contributions are pre-tax, and withdrawals for eligible expenses are tax-free.

Ascentek’s Medical and Dependent Care Reimbursement Accounts allow you to use tax-free dollars to reimburse yourself for a wide variety of health and/or dependent care expenses.

Eligible Expenses:

Rules and Regulations:

  • Plan your annual Medical Flexible Spending Account (FSA) contribution amounts carefully; the election you make when you enroll is binding for the entire plan year unless you have a qualifying life event.
  • You must incur eligible expenses during the plan year. Our plan year is from January 1–December 31.
  • If you incur fewer expenses than you expected, you may rollover up to $680 into the following year. This can be in addition to the IRS annual maximum. No action is needed on your part to rollover these funds.
  • You can’t transfer money from one account to another; money in your Health Care FSA can’t be used for dependent care expenses, and money in your Dependent Care FSA can’t be used for health care expenses.
  • You can only make changes to your contribution amounts for the Medical and Dependent Care FSA with a qualifying life event. Such changes include: marriage, divorce, birth or adoption of a dependent, or a change in employment status or health coverage of spouse or dependent.

Voluntary Pet Insurance

Our goal is to ensure every member of your family is protected, including your four-legged family members! You can purchase voluntary pet insurance to help with your pet’s medical expenses. This policy can be purchased at any time during the year. Premiums are paid through payroll deductions.

Tuition Reimbursement Program

Ascentek encourages teammates to expand their knowledge through education and specialized training. Tuition reimbursement helps you increase your effectiveness on the job while providing financial assistance.

Eligibility & Requirements

  • Full-time, part-time benefit eligible, and part-time teammates may apply.
  • Courses must be relevant to your current or future job requirements at Ascentek Services, LLC.
  • Prior written approval for reimbursement must be obtained from your Leader and the Senior Director of People & Culture before enrolling in the class.

Reimbursement: 100% of tuition cost upon satisfactory completion (grade “C” or better). Books, lab fees, and travel are not covered.

If you receive additional assistance from other sources, Ascentek will reimburse only the portion not covered by those programs.

Ascentek’s 401(k) plan helps you save for retirement with automatic enrollment and employer matching. You are eligible to participate starting the first of the month following 30 days from hire. The plan offers flexibility, tax advantages, and investment options to help you reach your retirement goals.

Ascentek Cares Fund (ACF)

The ACF is a teammate assistance program that exists to support the extended Ascentek family in
times of hardship and emergency. This fund is about keeping our promises to each other and the business
of families we love. This program is funded by Ascentek, teammates, and the Better Tomorrow Foundation,
and is administered by a 3rd party (Provision Bridge). The fund can be used to support teammates and their
families when they face medical challenges, home emergencies, memorial expenses, financial difficulties,

Igniter Fund Program

Giving back to the community is part of our DNA. Our teammate giving program—The Igniter Fund—is a
way for you to have ownership in our Community Promise. By donating to a 501(c)(3) of your choice, you can
double your gift to a nonprofit of your choice as Better Tomorrow Foundation will match your donation. You select
a 501(c)(3) you love from one of our categories—Health, Education, and Conservation—to accelerate our

Enrollment – How to Enroll


How to Enroll

To make your benefit elections for 2026, simply visit the Ascentek Connect portal at
ascentekconnect.com
and follow these steps:

  1. Enter username and password

    If this is your first visit to Ascentek Connect, you will need to login with a default password which is your date of birth
    (Ex. If your birthday is January 1, 1980 then your password would be 01011980).
  2. Navigate to “Menu” at the top left corner.
  3. From “Myself” select Life Events.
  4. Select the Description link of the applicable life event session.
  5. The enrollment page will appear.
